Masseter muscle hypertrophy (MSH) is a condition that occurs as a result of the masseter muscle, one of the muscles used to close the jaw, growing in volume. Especially when the teeth are clenched, the muscle becomes prominent on both sides of the jawbone and gives the face a square appearance. This is also called square face. In women, a square face can lead to a masculine appearance. This is the most important complaint about the square face appearance. Different treatment methods are applied for the treatment of masseter muscle hypertrophy, i.e. square face. One of these methods is to make an incision through the mouth and thinning the thickened muscle by shaving. Another square face treatment is Botox application to the chin. After the introduction of Botilinum Botox, Botox has started to be used for this purpose. Numerous publications have been made on Botox, book chapters have been written and treatment protocols have been established. The substance used in Botox application is a protein secreted by bacteria called Clostridium botulinum. Injection of Botox in certain doses into the muscle temporarily reduces or eliminates the contraction function of the applied muscle by blocking the electrical conduction from the nerves to the muscles. The effect starts on the 3rd-7th days after the procedure. Although it varies from person to person, this effect lasts for 4-6 months. If the patient benefits and is satisfied, it should be repeated in 6-month periods. It is a method that does not require anaesthesia. Night teeth grinding, also called bruxism, can lead to masseter muscle hypertrophy, jaw pain and fracture and deformity of the teeth. Jaw Botox is also used for the treatment of night teeth grinding, ie bruxism. It is stated that Botox application to the jaw reduces and improves symptoms such as head, jaw and toothache that often occur in the morning due to teeth grinding. For this purpose, teeth grinding treatment can be performed by injecting Botox into the muscle at 3-5 points with a special planning. In this treatment, Botox injection can be applied at intervals of 1-3 months until the desired result is obtained. The first application often gives results.

There may be redness, bruising, swelling and pain due to Botox injection. Rarely, subcutaneous and intramuscular bleeding may occur. Rarely, after the injection, Botox may leak into other muscles and nerves, leading to a decrease in muscle functions and especially weakness in the laughing function. There may be weakness in strong chewing. Sometimes in elderly people and those with high muscle volume, there may be loosening of the skin in the chin area due to decreased muscle volume after chin Botox, and pitting due to loss of volume in the lower part of the cheekbone.
Do not lie down for 4 hours after the procedure, do not massage the application site and avoid heavy exercise. This is to prevent the drug from being distributed to unwanted muscle groups and causing side effects. Avoid breastfeeding, pregnancy or choose an effective method of contraception.
After the planning as shown in the figure, Botox solution prepared with a needle is injected into the masseter muscle at 3-5 points. The procedure is painless. Rarely, Botox application to the chin may cause very mild pain. Ice application before the procedure prevents pain.
